Adult Transportation Waiver

a) A Candelen team member will contact you directly to confirm transportation. Due to distance limitations, signing the waiver below DOES NOT guarantee you will receive transportation services.

b) I grant permission for myself to be transported to and from Candelen activities by services hired by Candelen. As an adult, I remain legally responsible for any personal actions taken by myself. I hereby accept that the transportation to and from Candelen programs will be at my own risk. I agree to hold Candelen, as well as the hired service, harmless from any and all injuries, damage, or medical needs that may occur during transportation. Additionally, in order to be good stewards of the resource, I agree to have myself and the children I am responsible for ready no less than 5 minutes before the designated pick-up time. I also agree to be prepared with the appropriate car seat safety equipment required of each child or communicate in advance my need for car seats to be provided.

Child Care Transportation

a) I grant permission for my child(ren) to be transported to and from Candelen activities by either their Designated Child Care Provider or by services hired by Candelen.

b) As the parent and/or legal guardian, I remain legally responsible for any personal actions taken by the minor (“participant(s)”).  I hereby accept that the transportation of the minor to and from Candelen programs will be at my own risk. I agree to hold Candelen as well as the hired service harmless from any and all injuries, damage, or medical needs that may occur during transportation.

Child Care Guidelines

a) Candelen welcomes all to create a space where families, caregivers, and community can access resources, information, and supportive programming.

b) NOTE: Candelen and the places where services are administered are not licensed child care centers.

c) All families, caregivers, and children use the services of Candelen at their own risk and assume full responsibility for personal injury to self and family members, and further release Candelen for injury, loss, or damage arising out of the use of the facility or other third parties. Each child's parent or other legal guardian must provide consent and release prior to using Candelen's services for the first time.

d) Consent to attend child care is required from the parent or guardian of the child.

e) Candelen reserves the right to refuse entry to any person, or require any admitted person to leave the premises immediately, in the event that a member of Candelen’s staff determines (in his or her sole discretion) that the person's behavior (or the behavior of a companion of such person) is overly disruptive or aggressive, and violates any Candelen rules, policy, or is otherwise inappropriate. If that person is attending with a child, the child will also be obligated to leave. Children must also maintain appropriate behavior at all times. If a child does not behave appropriately, child care privileges may be revoked at the sole discretion of staff.  One violation is enough to suspend privileges. Any staff decisions to revoke privileges may be appealed by writing a letter to the Vice President of Programs.

f) Property is not to be removed from the facility. Violators will be assessed replacement costs for any lost or missing items. Families must assume responsibility for damaged items caused by neglect, reckless, or willful actions, and pay for replacement or repairs. Candelen is not responsible for any property brought into the space and does not maintain a Lost & Found.

g) Each child on site must be accompanied by the child's parent, legal guardian or Designated Childcare Provider. Each Designated Childcare Provider must be designated as such by the child's parent or legal guardian in a Parental Consent and Release.

h) All children must be signed in upon arrival and signed out when leaving. Your child(ren) are only permitted to leave with the person responsible for signing them in. In the event of an emergency during which the parent or Designated Childcare Provider is unable to pick up your child, the emergency contact provided on the information sheet will be contacted.

i) Children who are crawling (approximately 8 months old) and up to 10 years old are permitted in the child care space.

j) Candelen does provide snacks to children; however, outside snacks of any kind are not permitted due to life-threatening allergies and possible choking. Please only send water with the child and LABEL ALL CUPS.

k) SICK CHILDREN MUST BE KEPT AT HOME. If your child is taking any type of antibiotics, running a fever, experiencing stomach issues or vomiting, has pink eye, or a cough, they may not attend child care. Children cannot return until 24 hours after the time the fever breaks, vomiting ceases, or antibiotics have been started. You may also provide a doctor’s note stating it is permissible for the child to return if it is has been less than 24 hours or the child is still symptomatic. Candelen does not require or keep any records of immunizations on file. If any staff members notice any signs or symptoms of illness, we will immediately notify you or the Designated Childcare Provider and ask that the child be removed from the facility.

l) Candelen employees do not change diapers. If a child needs a diaper change, the person who dropped them off will be asked to return to the room. A changing table or diaper changing kit is provided.

m) All violations of these rules or similar incidents (injury, theft, inappropriate behavior, etc.) must be promptly reported to Candelen staff.

 

Child Care Assumption of Risk

a) The undersigned parent/guardian on behalf of my child(ren) agree that engaging in any program, including the supervision of my child(ren) during a Candelen program, shall be taken at the sole risk of the parent/guardian and child, including all consequential and incidental damages. The parent and child, for themselves and on behalf of their executors, administrators, heirs, and assigns, does hereby expressly release, discharge, waive, relinquish, and covenants not to sue Candelen (including its officers, agents, employees, and staff), hereby referred to as “Candelen” for all such claims, demands, injuries, damages, or causes of action, with respect to any Candelen program. The undersigned parent declares that their child is physically fit and able to participate in child care. The undersigned parent declares on behalf of their child that the child participates at their own risk.

 

Child Care Guidelines & Medical Services

a) I understand that in the event I violate any of the requirements or any of the Guidelines, or if I or my Designated Child Care Provider(s) or any child under our care or supervision otherwise take any action, deemed inappropriate by Candelen staff, I, and any children under our care or supervision, may be removed and barred further entry from Candelen programs.

b) I understand that Candelen does not provide health, accident or liability insurance to my child or me. I certify that I have adequate medical coverage for any medical services that may be required while my child(ren) are participating in activities with Candelen.

c) In the event that my child(ren) requires medical care, including surgery or administration of drugs, blood or anesthetic and I cannot be reached or the emergent circumstances do not allow time to reach me, I authorize each of the Designated Child Care Providers designated herein to obtain for my child such medical services as are deemed necessary. I understand and agree that Candelen does not assume any responsibility for any injury or damage which might arise out of or in connection with such emergency medical treatment.

 

Child Care Photo & Video

a) I understand that photographs, video recordings or audio recordings may be taken of my child(ren) during the participation in Candelen activities and I agree that Candelen, and those acting on their behalf, may use, publish and distribute any such photographs, video and/or audio recordings of my child(ren) and that I will not receive any payment or other compensation for such use. Further, I understand that such photographs, video and/or audio recordings, may be used in any media, including on the web and in printed materials, in connection with efforts publicizing, promoting or otherwise directly related to the objectives of Candelen.

 

Child Care Risks & Responsibilities

a) I agree to assume all the risks and responsibilities surrounding my child's use of Candelen’s services and participation in its activities and in advance release, hold harmless and, forever discharge Candelen and their respective governing boards, officers, agents, employees, (collectively, "Candelen"), from and against any and all liability for any harm, injury, damage, claims, demands, actions, causes of action, costs, and expenses of any nature which I or my child(ren) may have or may hereafter have, arising out of or related to use of Candelen services or participation in any of its activities, except if caused by the sole negligence of the any of the Candelen.

b) I have signed this release in full recognition and appreciation of the dangers, hazards, and risks of my child's participation in activities at Candelen, which could include serious injury and property damage. By signing this release, I acknowledge and represent that: I have fully informed myself of its content and meaning and I understand it; and I am signing it voluntarily and with the intent to be bound. I further agree that this release shall be construed in accordance with state laws without regard to conflict of laws principles.
